In order to begin the process, we ask that families complete the Face Sheet of the Application and their autobiographies. One autobiography is required for each adoptive parent. These can be emailed, faxed, or mailed to us. Once we receive these documents, we can schedule a home visit, normally within two weeks. TO DOWNLOAD ALL FORMS AT ONE TIME, we offer a .ZIP version.
Face Sheet
Caring Adoption Associates requests the basic file information sheet to be completed prior to scheduling your in home visit. The Face sheet has the contact information and family overview information.
Autobiography
The autobiography is to be completed by each adoptive parent. The autobiography is an important tool which enables your social worker to get to know you, your family and more about your plans to adopt. The following is an outline of topics that we would like you to cover. We recognize that all people have both negative and positive experiences throughout their lives and we ask that you share this information with us as openly as possible.
Financial Information
Caring Adoption Associates requests completion of the financial information form. Basic income, expense, savings, debt and investments are considered for the ability to care for adopted children.
Home Study Questionnaire
Caring Adoption Associates requests completion of the Home Study Questionnaire. Basic Q&A of previous adoptions and foster care placements, criminal background and mental health questions.
Medical Form
Please download one medical form for each adoptive parent. Lab test are required. You may be able to call your doctor to schedule the labs, then have the form completed at the time of your physical.
Reference Form
